PCP to Specialist Referral

Please Note: Sterling Family Practice is not contracted with insurance companies and cannot provide insurance-required referrals. We can only provide a PCP to Specialist referral.

What You Need to Know:

  • Check with Your Insurance: Contact your insurance provider before scheduling with a specialist to:

    • Confirm if a referral is required

    • Ensure the specialist is in-network with your plan

  • Referral Requests from SFP: To request a PCP to Specialist referral you can send your request to info@sterlingfamilypractice.com we will need the following information to process your request.

    • Specialist name

    • Address

    • Telephone

    • Diagnosis or reason for seeing the specialist

Processing time: Please allow 3–5 business days You must have been seen at SFP within the past 12 months.

If needed, we may ask you to schedule an appointment with one of our providers before issuing the referral
