  • Yes. As family practice providers, SFP cares for people of all ages, whether they’re young in years or young at heart.

  • Yes! We look forward to partnering with you in the health and wellness of your child.

  • Beginning November 1, 2024, our fee structure is the following:

    • Initial Paperwork for New Patient: $45

    • Well Child (0-17): $165

    • Well Adult (18-39): $185

    • Well Adult (40-64): $195

    • Well Adult (65+): $225

    • Follow-up Televisit (short): $85

    • Follow-up Televisit (longer): $165

    • Sick Visit or Routine Medication Refill: $135

    • New Problem Visit: $165

    • Complex Visit: $225

    • Osteopathic Visit: $225

    • Biofield Visit: $350

  • Yes, you will be able to use your HSA or FSA card to pay for your visit at the time of service.

  • Starting November 1, 2024, Sterling Family Practice will not participate with insurance carriers apart from Medicare. This change allows us to serve you directly.

    All patients apart from those using a Medicare plan, will be provided with a superbill which they can submit to their insurer for potential reimbursement of services.

  • Yes, we will continue to see and bill insurance for Medicare primary patients. If you have a Medicare Advantage Plan, or you have a secondary insurance plan, we will not bill those companies. You can submit a claim with your superbill for reimbursement.

  • Beginning November 1, 2024, Sterling Family Practice will be out of network with all insurance carriers except for Medicare.

    After your visit, you will have access to a superbill that you can mail to the claims address on the back of your insurance card with your insurance's designated claim form.

  • Reimbursement will be dependent upon both your insurance carrier and your individual plan and out-of-network deductible.

  • Functional medicine is an integrative approach to health and wellness through individualized, science-based analysis that compliments conventional medicine. Providers who have additional training in functional medicine look for the root cause of an illness in a person, and then address the whole person, rather than the isolated symptoms.
